Certified Community Health Worker (Central Indiana)

The Community Health Worker is responsible for proactively identifying ways to improve the health of our members and meet quality goals while building relationships in the community that will assist in the removal of socioeconomic barriers.

The Community Health Worker serves as members of the care delivery team; the main responsibilities include, educating individuals in managing their own health, assist them in navigating the health care system, connecting these members with healthcare providers; and, help them access community resources in order to overcome socio-economic barriers that might impact their health outcomes. Additionally, the Community Health Workers help improve health care access and outcomes, strengthen health care teams, and enhance the quality of life.

Positions require extensive travel and working within a specific regional area.


High School Diploma or equivalent GED. (Associate Degree preferred) AND Four (4) years of related experience; or any combination of education and experience, which would provide an equivalent background. Basic knowledge of community-based populations, medical conditions and/or disabilities. Must be willing to develop relationships with local communities and have strong presentation, interpersonal and communication skills. Must have good organizational skills and be able to work independently and effectively with others. Ability to handle protected health information and personally identifiable information in accordance with HIPAA requirements.

Community Health Worker Certification is preferred but must be obtained within 90 days of employment. Valid Driver’s license and access to a motor vehicle with valid motor vehicle insurance required. Bi-lingual candidates (Spanish and Burmese) are encouraged to apply!
