Job Summary

The Community Health Worker is responsible for ensuring that appropriate member treatment plans are followed on complex cases and for proactively identifying ways to improve the health of our members and meet quality goals. Positions require extensive travel and working within a specific regional area.Must have experience working with Medicaid/disability and homeless clients.

Responsibilities and Duties

MINIMUM REQUIREMENTS High School Diploma or equivalent GED. (AD preferred) ANDFour (4) years of related experience; or any combination of education and experience, which would provide an equivalent background. Working knowledge of community based populations, medical conditions and/or disabilities. Must be willing to develop relationships within local communities and have strong presentation, interpersonal and communication skills. Must have good organizational skills and be able to work independently and effectively with others. Ability to handle protected health information and personally identifiable information in accordance with HIPAA requirements. Certified nurse assistant or certified medical assistant is preferred.Community Health Worker Certification is preferred but must be obtained within 90 days of employment.

Valid Driver’s license and access to a motor vehicle with valid motor vehicle insurance required. Bi-lingual candidates (Spanish and Burmese) are encouraged to apply!
